Thank you for registering for Walker’s All For Girls Summer Academy! In order to ensure the safety and well-being of all of the girls joining us this summer, we need to make sure all related permission and consent forms are filled out, signed, and returned prior to your student’s arrival.

The following forms must be completed by the parents or guardians of participants in our grades 5-8 in-person program. Please download, complete, and return scanned forms digitally to Erin Ott at eott@ethelwalker.org or mail the forms to the below address. The Health Exam Form must be completed within 12 months of the program.
